Anal glands, or anal sacs, are two small glands present in both dogs and cats situated just below and either side of the anal opening. These glands produce a substance that is secreted through ducts just inside the anus when pressure is put on the glands – most commonly by the passing of feces.

When I first received the product I used it every other day for about a week, just to help things settle down back there. Then I switched to roughly twice a week application. Sometimes I forget and it drops down to once a week. We used to have to empty her anal glands every 4-8 weeks or so, or they would get extremely full, and sometimes even infected before she would show me any signs that there were problems back there. We've now gone 4 months without having to manually express them. Also if you're looking at the product thinking there's no way your dog will allow a compress on his back side, it really doesn't have to be on there that long. I take a little cup of the dog's kibble to the bathroom with us and slowly dole out the treats while applying the product.
You might also be thinking the product is super expensive. It is an investment, but this little bottle is still 2/3 full after 4 months, so we may just get a year's use out of it. I use 5 drops in a quarter of a cup of water each time for a beagle, larger dog may need more.
Edited to add - 6 months later and we still haven't had to empty the glands! Still using 1-2x per week, very occasionally using more often if I see the dog scooting.
